There are a few ways this could be done...
1: link to a public wiki page that is user editable and essentially just has a table of ports and apps.
2: link to a document using some form of share point or otherwise allows for public access (perhaps within the developers section as this would limit editing to only registered devs)
3: create a new read with a master table in the first thread... Developers can add ports in new posts and then periodically the OP can be edited to include the newly added ports.

I'm sure there are more ways to do the same....
